NDIS Plan Review: How to Request Changes to Your Plan

An NDIS plan reassessment may be needed when your circumstances or support needs have changed significantly and your current plan no longer meets your needs. From 27 August 2026, new rules apply to how participants can request a plan reassessment outside their scheduled reassessment date. Under the current NDIS rules, only the participant, their plan nominee or their child representative can request an unscheduled plan reassessment. The request must explain what has changed, why a reassessment is needed and include supporting evidence where required.

For smaller, urgent or short-term changes, a plan variation may be more appropriate than a full reassessment. The NDIS can decide whether to reassess your plan, vary your plan or not make a change after considering your circumstances and evidence.
In this guide, we explain when you can request an NDIS plan reassessment, what has changed under the 2026 rules, what evidence you may need and what happens after you submit your request.

What Is an NDIS Plan Review or Plan Reassessment?

An NDIS plan reassessment is the process used when your support needs or circumstances have changed significantly and your current plan no longer meets your needs. A reassessment can result in a new plan with different NDIS supports. A plan variation is different. It is used for smaller, urgent or short-term changes that can be made without a full reassessment. The NDIS may vary your existing plan when a full reassessment is not needed.

When Can You Request Changes to Your NDIS Plan?

You do not always have to wait for your scheduled review to make changes to your NDIS plan. If your situation changes, you can request a plan review at any time. Life circumstances can shift unexpectedly, and your supports should reflect those changes. You can request a plan reassessment when there has been a significant and ongoing change to your circumstances or support needs and your current plan no longer meets your needs.

From 27 August 2026, only the participant, their plan nominee or their child representative can request an unscheduled plan reassessment. A provider cannot request an unscheduled reassessment on your behalf.

Examples of changes that may support a reassessment include:

  • A significant change in your functional capacity or support needs
  • A significant change in your living arrangements
  • A significant change affecting your education or work
  • A significant change in the informal support available to you
  • Your current plan no longer meeting your disability-related support needs

You need to explain what has changed and provide relevant evidence to support your request. The NDIS says evidence may include recent reports, assessments or other information from treating healthcare professionals, therapists or support workers.

If the change is urgent, minor or short-term, a plan variation may be more appropriate than a full plan reassessment.

How to Request an NDIS Plan Reassessment or Plan Change

If you need a significant change to your NDIS plan outside your scheduled reassessment, you now need to complete the NDIS plan reassessment request form.

Your request needs to explain:

  • What has changed
  • How the change affects your support needs
  • Why your current plan no longer meets your needs
  • What evidence supports the change

The request must come from you, your plan nominee or your child representative. You cannot give a provider permission to request an unscheduled plan reassessment on your behalf.

The NDIS may require recent evidence from healthcare professionals, therapists or support workers. The current NDIS guidance says evidence will generally need to be recent, usually within four months of the reassessment request and after the current plan was approved.

Once the NDIS has the information it needs, it has up to 90 days to decide whether to reassess your plan, vary your plan or not make a change.

What Changed for NDIS Plan Reassessments in 2026?

New NDIS laws changed the plan reassessment process from 27 August 2026.

The main changes include:

  • Only participants, plan nominees or child representatives can request an unscheduled plan reassessment.
  • Requests must be made using the appropriate form.
  • Participants need to explain what has changed and why a reassessment is needed.
  • Supporting evidence may be required.
  • The change must generally involve a significant and ongoing change to support needs or circumstances.
  • The NDIS has up to 90 days to decide whether to reassess the plan, vary it or not make a change.
  • Plan variations remain available for urgent, minor or short-term changes.

These changes apply to requests for an unscheduled plan reassessment. Participants can still continue using their current plan while the NDIS considers their request.

What Happens After a Plan Review ?

fter the NDIS receives your plan reassessment request and the required information, an NDIA planner will consider your current plan, the evidence you provide and the changes you have requested.

There are three possible outcomes:

  1. Your plan is reassessed and a new plan is created.
  2. Your plan is varied if the changes can be made without a full reassessment.
  3. Your plan is not changed if the information and evidence do not support a reassessment or variation.

The NDIS says it will make a decision within 90 days once it has the information it needs for a plan reassessment request. You will be told the outcome and the reasons for the decision.

If you disagree with an NDIS decision about your plan, you may have the right to request an internal review of the decision.

Common Reasons for Plan Reviews

There are several common reasons why participants request an NDIS plan review. In most cases, it happens when their current plan no longer reflects their needs or daily situation. As life changes, your supports should change with you. Recognising these situations early can help ensure you continue receiving the right level of support. Some of the most common reasons include:

  • Significant changes in functional capacity or support needs
  • Changes in living arrangements
  • Changes in education or employment
  • Changes in informal supports
  • Your current supports no longer meeting your disability-related needs
  • Significant changes in your circumstances that affect how you need to use your NDIS plan

In many cases, participants realise they need a review when they start facing difficulties in managing everyday activities or achieving their goals with their current plan. Understanding these common reasons can help you identify when it may be the right time to request a review and ensure your supports remain suitable for your situation.

How to Prepare for Your NDIS Plan Reassessment?

Keep relevant reports and evidence up to date, consider how your circumstances have changed and make a clear record of the supports you currently use. If you are approaching your scheduled reassessment, your my NDIS contact will generally contact you around 2 to 3 months beforehand to discuss your plan and what information may be needed.
